As part of the curriculum residents are required to report adverse events, near misses, or unsafe conditions that they observe or are a part of in the day-to-day care of their patients.  Reports submitted by the residents are forwarded to the quality and safety departments of the hospital or clinic where they occurred and the interns/residents have opportunities to participate in the improvement activities related to their report.


Adverse Event: An Injury that was caused by medical management rather than the patient’s underlying disease.

Medical Error: The failure of a planned action to be completed as intended or the use of a wrong plan to achieve an aim.

Serious Error – Error that causes permanent injury or transient but potentially life-threatening harm.

Minor Error – Error that causes harm which is neither permanent nor life-threatening.

Near Miss – An error that could have caused harm but did not either by chance or timely intervention.
